IQ HEALTH PORTAL IS AVAILABLE TO ALL BELKYS BRAVO'S PATIENTS TO VIEW AND UPDATE THEIR MEDICAL INFORMATION, AND CONNECT WITH THEIR HEALTHCARE TEAM.

If you have not done so already, please make sure that during your next visit you ask to receive a personal invitation to IQ Portal and you will be able to see all your visit summaries, Medications, Allergies, Lab results and personal information in a secure, efficient and easy-to-use manner.

IQ Portal also offers you the ability to Communicate thru Direct email with our office for any Refills, Medication questions, Change of Insurance or Address, PLEASE NO URGENT REQUEST OF ANY KIND.

Our Office is open to assist you Monday-Friday 8am-6pm and can answer any questions that need immediate attention.
